Parents And Student Assessments
Our individualized assessments are designed for parents, not children, to complete.
We are requesting each parent (or parents) to complete the assessment before meeting with our consultant. Generally, our consultant will discuss the finding of the asssessment in the inital interview and continue the discussion as we work with your child, adolesecent or young adult.
Our parent portal allows each parent to complete information needed during the assessment process. The link below goes directly to our portal and needs to be completed by the parent
Parents Assessment For Traditional Boarding School
This assessment is to be completed by the parent(s) looking at a traditional boarding or day school in the United States or Canada.
Parent Assessment For Special Needs Student
This assessment is to be completed by the parents of a child with special needs looking for a therapeutic school placement.
Parent Assessment For International Student Placement For Boarding School
This assessment is to be completed by parents looking for a boarding school placement in the United States. Also applies to parents looking at summer boarding or day school.
Parent Assessment For Charter Schools
This assessment is to be completed by parents looking for a charter school.
Our Parent Letter
This letter is a sample letter for parent (s) which explains our services. The letter tells parents where to send the documents we request and how to contact us after the letter is read.
Parent Agreement For Services
This is a sample agreement for parent (s) to view. The parent (s) interested in our services will receive an individual copy of this agreement by our educational consultant, along with a request for an electronic signature and approval by email.
Parent Assessment For Transition
This assessment is to be completed by the parent who has a child currently in a therapeutic school or wilderness program looking at either leaving or moving the child to a different program.
Parent Assessment For Military Boarding School
This assessment is to be completed by a parent or parent(s)looking for a military boarding school.
Acute Care Assessment For Young Adults
This assessment is to be completed by a physician or medical professional for young adults (18-24) with mental illness or need for placement in a residential treatment program.
Parent Assessment For Children Or Young Adults With A Developmental Disability Or Challenge
This assessment is to be completed by a parent (or parents) who has a child above the age of five or older with a developmental disability or mental health issues with below 70 in IQ.
Student Assessment For College Placement
This assessment is to be completed by the student looking for a college placement before graduation from high school.
Assessment For IEP
This assessment is to be completed by the parents of a student who either has an existing IEP or 504 plan.
Parent Assessment For Drug And Alcohol Placement
This assessment is to be completed by the parent(s)of a student with a drug or alcohol dependency needing treatment or recommendations by our consultant.
